Exit Pathway
Review patient daily to assess suitability for community IV Therapy

Antibiotic plan discussed and agreed with infection specialist
(not necessary for clinically stable cellulitis patients)

Contact appropriate Home IV team for patient to be accepted
(Solihull, Staffordshire or Birmingham cluster)

Consider vascular access device
<9 days of IVs cannula sufficient
>9 days requires mid/PICC line

Documentation to be sent home with the patient:
– Signed assessment criteria (referral form)
Preprinted prescription
– Discharge letter to GP (inpatient only)
– Information leaflets as appropriate
